The Theory Was Completely Wrong. The Drains Were Completely Right.

In the 1850s, London smelled extraordinary. Not extraordinary in a way anyone enjoyed – the Thames was essentially an open sewer, the streets were thick with horse dung and rotting rubbish, and the basements of the poorest neighbourhoods sat damp and reeking in permanent shadow. Cholera kept arriving, killing thousands, vanishing, returning. And the doctors of the day had a perfectly confident explanation: miasma. Bad air. The poisonous vapour rising from all that filth was getting into people's lungs and killing them.

They were completely, embarrassingly wrong. And their wrongness saved an enormous number of lives.

The Smell Is the Danger (No, Wait…)

Miasma theory had been around since antiquity – the word itself comes from the Greek for pollution or stain. The logic seemed airtight, if you'll forgive the word: rotting things smelled, sick places smelled, therefore the smell was the sickness. Get rid of the smell, get rid of the disease. When cholera swept through cities, the miasmatists pointed at the reek rising off the Thames and said: there's your culprit. Fog was dangerous. Night air was dangerous. Marshes were practically death itself in vapour form.

And so Victorian public health officials set about doing something about it. They drained marshes, pushed for clean water supplies, tore out the cesspits to build sewers. They ventilated hospitals, opened windows, scrubbed wards, removed the accumulated filth of decades. All of it aimed at defeating the miasma, the foul air, the entirely fictional mechanism they had decided was killing people.

Meanwhile, the thing that was actually killing people – Vibrio cholerae, a bacterium spreading through contaminated water – was being removed from the water supply. Not because anyone knew it was there. Because clean water happened to be what you got when you chased bad smells away from wells.

The Man Who Was Right for the Wrong Reasons

John Snow is often held up as the hero who cracked cholera – the epidemiologist who mapped the 1854 Soho outbreak, traced it to the Broad Street water pump, and proved that cholera spread through water, not air. He deserves the credit. But even Snow, doing genuinely brilliant detective work, was operating without germ theory. He didn't know about bacteria. He just knew, from careful observation, that the pattern pointed at the water, and the microscopic why came later.

The point is that the practical recommendations of the miasmatists and the germ theorists ended up being almost identical. Clean water. Proper drainage. Remove waste. Ventilate spaces. Don't let filth accumulate. The losing theory and the winning theory issued very nearly the same instructions – they just had completely different stories about why those instructions mattered.

The death rate didn't wait for the argument to be settled. The drains worked either way.

The Gap Between Why and What

This is the part that should probably make us a bit humble. Germ theory – developed by Pasteur and Koch in the latter half of the 19th century – was a genuine revolution in understanding. It opened the door to vaccines, antibiotics, everything that followed. Understanding the actual mechanism wasn't just a satisfying footnote. It mattered enormously, eventually.

But here's the thing: between the wrong explanation and the right one, people were already doing the right thing. There's a version of history where the miasmatists, embarrassed by their incorrect theory, had recommended something useless – burning aromatics, perhaps, or wearing protective herbs. They didn't. This gap – between knowing that something works and knowing why – turns out to be pretty common in medicine. Aspirin was in widespread use for decades before anyone understood the mechanism. The cholera story isn't just an interesting bit of Victorian history. It's a small corrective to the assumption that understanding always precedes improvement. Which doesn't mean explanations don't matter, or that you should trust any confident-sounding theory that happens to recommend sensible-looking behaviour. The miasmatists got lucky; their wrong idea happened to point at real filth. A different wrong idea could have pointed somewhere useless entirely.

It just means that in medicine, "this works" and "here's why it works" have always been two separate discoveries – and the second one sometimes arrives quite a long time after the first.
